Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026. The Team & Role Access, is the first point of contact for information and referrals to a wide range of community health, rehabilitation and specialist services provided by Bayside Health – Peninsula Care Group. This position is responsible for ensuring timely referral management and responsive contact with clients, connecting them with the most appropriate service, as per client needs and Peninsula Care Group service guide criteria. Key duties and responsibilities • Develop a strong knowledge of services provided by the Peninsula Care Group and within the community sector and provide relevant information to consumers • Assist with the triaging of referrals to identify priority, ensuring timely referral management. • Contact referred clients/representative to confirm they are aware of the referral and consent to the referral proceeding • Collect/check demographic data as required and enter into iPM and UNITI • Conduct initial needs identification with the specific purpose of clarifying the clients’ needs, eligibility, suitability risk and priority for all service referrals • Provide information to clients about the service and/or advice on alternative service options (as per service guide) where appropriate • Develop and maintain an up-to-date knowledge of funding eligibility and provide information to clients regarding the service fees (where relevant) in alignment with the Community Health and Rehabilitation, Ageing, Pain & Palliative Services – Schedule of Fees Guideline • Ensure accurate booking of client appointments, where required Skills & Experience Required Essential • A relevant tertiary qualification and/or significant relevant experience in the health or community health services sector • Advanced interviewing and engagement skills • Proficiency in the Microsoft Office suite, particularly MS Word and other data base software programs • Demonstrates advanced computer and keyboard skills • Highly developed verbal and written communication skills/ Desirable • Sound knowledge of relevant health and community services • Experience in a phone based intake role, responsible for screening individuals to determine eligibility, risk and priority for a service.
