The Role As a Support Coordinator, you will: Lead the client intake and onboarding process, including service agreements, care plans, and gathering relevant medical information. Coordinate supports for NDIS participants, helping them achieve their goals and make the most of their plans. Maintain accurate, up to date client records and case notes, while ensuring strict confidentiality. Build and nurture strong relationships with clients, families, support providers, and external stakeholders. Guide participants in selecting suitable service providers and connecting with community resources. Monitor client progress, prepare reports, and manage administrative tasks to support high-quality service delivery. Ensure all work complies with organisational policies, NDIS standards, and safety protocols. Support participants to build independence, develop skills, and achieve their personal goals. About You To be successful in this position, our ideal candidate: Minimum of 12 months’ experience as an NDIS Support Coordinator. Relevant qualification in community services, counselling, welfare support, disability support or social work. Is proficient in all Microsoft Office applications. Proven case management experience, or transferable experience within the human services sector. Strong understanding of community resources and disability rights. Knowledge of the NDIS, with demonstrated experience in NDIS Support Coordination highly regarded. Currently holds, or is willing to obtain, a driver’s licence, access to their own vehicle, First Aid and CPR certificates, NDIS Worker Screening Check, completion of mandatory NDIS training modules, Police Check, and Working With Children Check (WWCC). The Essentials Strong interpersonal and communication skills, with the ability to build rapport and work effectively with participants, families, and service providers. Excellent organisational and time management skills, with the ability to prioritise tasks and manage multiple participants simultaneously. High attention to detail and accuracy.
