The Casual Care Partner provides person‑centred care management within the Support at Home Program, ensuring older people continue to live safely and independently at home. Working flexibly across the service, this role steps in as needed to cover staff leave or operational demands—maintaining continuity of care, coordinating support, and upholding wellness, reablement and culturally safe practice across all care pathways.
