In this leadership position, you will oversee the delivery of a high-quality Transition Care Program that supports older people at a critical point in their recovery journey — bridging the gap between hospital and home or residential aged care. You will lead a multidisciplinary team to provide short-term, goal-directed and therapy-focused support for people aged 65 years and over, helping them: Regain functional independence Avoid unnecessary extended hospital stays Reduce premature entry into residential aged care Make informed long-term care decisions This is a pivotal role at the interface of acute, sub-acute and community care. You will ensure safe, coordinated transitions, optimise functional outcomes, and uphold best-practice care planning in line with the Transition Care Program guidelines.
