About the role Mater has an exciting opportunity for an experienced Clinical Nurse to work within the Hospital in the Home (HITH) Nursing Team to provide acute clinical care to patients in their homes. This role is offered as 6 month max term contract on a part time basis. The Hospital in the Home (HITH) Clinical Nurse is a dynamic and self-directed role providing advanced clinical competency to HITH patients. HITH operates to deliver hospital-level care in the safety and comfort of a person’s residence. Using extensive experience, this role is responsible for the coordination and delivery of acute and post-acute care in the community. Salary range: $54.44 - $58.30 per hour plus superannuation and access to not-for-profit salary packaging up to $15,900 access to many staff benefits! What you’ll be doing Key responsibilities for this role include: Achieving optimal patient outcomes throughout the patient journey and continuum of care from referral through to outpatient follow up and documentation of accurate patient records Demonstrating advanced clinical expertise and knowledge of procedures and the management of HITH patients, including proactively seek clarification when care requirements are uncertain or additional care is required through referral to multidisciplinary team Proactively identifying and supporting continuous quality improvement and risk management strategies ensuring integrated and responsive HITH service provision Maintaining currency of clinical knowledge and skills and achievement of new clinical competencies as required so as to deliver new treatment protocols as the service expands Commitment to fulfil professional development objectives as determined in professional development plan Maintenance of corporate and clinical mandatory training in line with the service’s key performance indicators Commitment to impart knowledge and share skills through supervision of nursing students and mentorship of less experienced team members Exceptional time management and self-management skills as required for autonomous workload management, including the ability self-identify fatigue and observe strategies to ensure driver safety Excellent stewardship of Mater resources maximising patient access to nursing services, use of consumables and fleet management About you To be successful in your application, you will have: Current AHPRA registration as a Registered Nurse (no restrictions) At least 3 years’ experience as a Registered Nurse Current open driver’s licence Post graduate qualifications in a related discipline is desirable but not essential Previous experience as a Clinical Nurse, liaison officer or navigator position highly desirable Excellent written and verbal communication skill and the ability to liaise with a variety of different stakeholders both internally and externally at various levels Demonstrated ability to operate in a team environment and autonomously using initiative, sound reasoning and strong listening, negotiation, and decision-making skills Strong attention to detail and organisational skills Commitment to ensuring a responsive and streamline HITH service is always delivered to patients.
