This role is based onsite in our Osbourne Park or Jandakot clinic, where you will be the welcoming face and confident first point of contact for participants, families and referrers from initial query, to service delivery, to supporting NDIS participants get more out of life today! On a daily basis, you will: Allocate referrals quickly and accurately so participants can access services without delay Convert enquiries into referrals by providing clear, practical information Guide participants and families through our services and the NDIS with confidence Work closely with clinicians and leaders to support smooth scheduling and service delivery Complete funding checks and ensure NDIS funds are available for service Manage new and existing funding requests to ensure NDIS funds are available for service Provide high quality and individually tailored advice to participants who are navigating the NDIS Ensure robust case management activity with all activity entered into the system, ensuring all progress, plans, communications and interactions with participants and stakeholders are clearly documented Manage bookings, reminders, emails and follow up calls Welcome clients to the clinic and support all front of house duties Support day to day clinic operations The ideal candidate will have the following: Ability to commence from the April 2026 A sound understanding of person-centered thinking, the principles of the NDIS and the National Disability Service Standards, with experience working within the NDIS and/or disability sector Experience in an intake or similar role (desirable) NDIS Worker Screening Clearance & Working with Children Check Case management or related experience Exceptional customer engagement skills and experience, delivering high quality, individually tailored service Highly developed communication and negotiation skills Ability to work independently, set own priorities, work to targets and respond to a changing environment Proficient in PC based applications e.g. Microsoft Office, database applications Ability to understand and work within prescribed systems and procedures
