Our services have a strong focus on prevention and early intervention and enable access to safe accommodation options, Holistic case management, community outreach programs and programs that teach vital independent living skills. About the role As a Case Manager , this is your opportunity t o deliver high quality case management support services to young people 16-25 years of age including families with accompanying children who are at risk of or experiencing homelessness. Reporting to the Client Services Coordinator, this is a permanent part-time role offering 30.4 hours per week , located in Ringwood, VIC . How you will make an impact Provide client-centred case management in line with the TSA Youth Services Model of Care, building effective relationships that promote self-determination, participation, safety, inclusion (including LGBTIQA inclusion), and trauma- and psychologically informed practice. Undertake ongoing assessment from intake to exit, identifying risks including suicide and non-suicidal self-injury, and develop, implement and regularly review collaborative case, safety and housing plans (at least every three months) in accordance with policy and procedure. Support clients to access and navigate community and specialist services through referrals and advocacy, identify service gaps, contribute to service improvement and innovation, and represent TSA Youth Services in regional networks to advocate on youth homelessness and strengthen pathways. Maintain accurate , timely and compliant client records using TSA information systems, ensuring case notes, assistances and documentation are completed within 24 hours and WHS and risk matters are escalated appropriately. Work under the direction of the Client Services Coordinator and State Practice Lead, complete weekly youth outpost delivery within the Eastern region, and undertake additional duties as within agreed timeframes . What you will bring Degree qualification in the area of Social Work, Social Welfare or related field and demonstrated experience with accompanying highly developed skills in working with youth experiencing homelessness and who display a range of highly challenging behaviours is essential. Knowledge of the service systems, in particular youth, housing and homelessness, Child Protection, AOD, Family Violence and Mental Health Understanding and experience in applying Case Management principles and practice and willingness to assist in the provision of support in the areas of youth homelessness, family violence, drug & alcohol, mental health, and Life and living skills.
