The Role As our Care Partner – Support at Home, you'll be the architect of personalised care journeys for our valued clients. This is a strategic coordination role requiring strong industry knowledge, sound judgement, and the ability to adapt as the aged care landscape continues to evolve. You'll: Be the primary advocate and point of contact for clients across Support at Home, Fee for Service, and Brokerage programs Design and manage comprehensive, individualised care plans aligned with the Aged Care Act 2024 and Strengthened Aged Care Quality Standards Collaborate closely with our Clinical and Allied Health teams to deliver exceptional wellness and reablement outcomes Strategically manage quarterly budgets, maximising utilisation through proactive planning and innovative service delivery Champion client choice, independence, dignity, and rights in every interaction Navigate ongoing sector reform with confidence, contributing to continuous improvement across the team What You'll Bring This role requires someone who brings both experience and adaptability. You'll need: Demonstrated coordination experience in aged care, preferably in-home or community services — this is essential, not negotiable Resilience and comfort with change — you've worked through sector reforms, shifting funding models, or organisational change and you've thrived, not just survived Certificate IV in Aged Care, Community Services, or Individual Support, and/or a relevant tertiary qualification (a clinical background is highly regarded) Strong understanding of aged care funding frameworks, particularly Home Care Packages and the Support at Home program Excellent communication and relationship-building skills with clients, families, and multidisciplinary teams Sound judgement, initiative, and the ability to manage competing priorities A genuine commitment to enhancing the lives of older Australians Current National Police Clearance (within three years) Australian citizenship or valid visa with full working rights Why Curtin Heritage Living?
