Our teams walk alongside adults and families, offering accommodation, case management, outreach, financial assistance, community connection and referral pathways. The Open House Program provides supported accommodation and psychosocial support for people experiencing homelessness, including those navigating mental health challenges or the Justice system. We focus on safety, stability and self-determined goals, empowering people to rebuild their lives with dignity. About the role As a Case Manager, you’ll be part of a passionate team supporting residents to create meaningful change in their lives. You’ll work collaboratively with each person to understand their story, identify their strengths and goals, and develop practical, achievable case plans. Your days will be varied and purposeful, from completing assessments and safety planning, to advocating for residents, coordinating supports, and building strong relationships that encourage participation and independence. This is a permanent full-time role based in Darwin, reporting to the Open House Team Leader. Pay is in accordance with the SCHADS Award Level 4. How you will make an impact Provide person centred case management that supports residents to achieve their goals Undertake intake and assessment to build a shared understanding of needs, strengths and risks Maintain accurate, up to date client records in line with TSA standards Support residents to attend appointments and advocate on their behalf with government and community services Identify and respond to potential high-risk situations in line with policy and procedure Build effective, respectful relationships that promote safety, participation and self determination Work collaboratively with internal programs and external partners to ensure coordinated support What you will bring Degree qualification in Social Work, Mental Health, Welfare, Community Development or related field; OR a minimum Certificate IV in Community Services with relevant experience Experience supporting people involved with the Justice or Corrective Services systems Understanding of homelessness, alcohol and other drug issues, mental health and complex needs Experience working with people from culturally and linguistically diverse backgrounds, including Aboriginal communities Strong organisational and time management skills Ability to maintain professional boundaries, self-care and resilience NT Driver’s Licence First Aid (or willingness to obtain) Willingness to participate in an on-call roster What we offer The Salvos offer a meaningful and balanced benefits package, including: Salary packaging up to $15,900 tax free and $2,650 meal and entertainment benefit Generous paid parental leave (12 weeks primary carers) Purchased Leave Scheme (up to 8 weeks) 5 days paid leave annually to experience other TSA programs Flexible working arrangements Access to EAP and wellbeing initiatives Ongoing training and development to support your growth Purpose driven work that creates lasting social impact How to apply If you’re passionate about supporting people to build stability, independence and connection, we would love to hear from you.
