We deliver practical, person centred supports across home and community settings, with a strong focus on dignity, inclusion, and quality of life. About the role We are seeking a motivated and compassionate Community Worker to join our team on a full time, permanent basis. This role involves working directly with clients to support community access, daily living, and social participation. You will play a key role in helping clients build independence, connect with services, and engage confidently in their communities. Key responsibilities • Assess individual needs, strengths, and goals through ongoing engagement with clients • Plan and deliver personalised community based support programs • Support clients to attend medical appointments, shopping, banking, and daily activities • Facilitate access to recreational, social, and cultural activities • Liaise with families, carers, and service providers to coordinate supports • Monitor client progress and adapt support strategies as needs change • Provide information, advocacy, and referrals to relevant community services • Maintain accurate case notes, reports, and service documentation • Deliver supports respectfully in home, group, and community settings Skills and experience • Minimum two years of experience in disability, community services, aged care, nursing, or mental health support roles • Strong communication and interpersonal skills • Ability to work independently in the community and as part of a supportive team • Confidence in managing diverse client needs and maintaining professional boundaries • Sound organisational and reporting skills Qualifications At least Diploma in community services or three years of full time works experience in NDIS setting Benefits • All outing activities are free for staff, including cinema, bowling, events, and shows • Engaging and social work environment with daily outings and varied activities • Friendly, inclusive, and supportive workplace culture • Opportunity to take home a company vehicle, subject to role requirements • Career development opportunities through training, promotions, and pay progression • Excellent client to staff ratios • Access to in house and online training and education Requirements • Current Australian driver licence and access to a personal vehicle willing to transport clients, with travel reimbursed at $1 per kilometre • Current First Aid and CPR certificate, or willingness to obtain • Willingness to work rostered hours, including weekends • Ability to pass NDIS Worker Screening Check, Working with Children Check, and National Police Check • Willingness to complete mandatory NDIS online modules • Willingness and ability to provide personal care to both male and female clients when required • Ability to remain calm under pressure and respond effectively in challenging situations If you are passionate about supporting people to live active, connected, and meaningful lives, we encourage you to apply and join ACME Support Services as a Community Worker.
