Make a meaningful difference with UnitingCare Queensland We are seeking an experienced Outreach Case Manager to provide specialised, community-based case management support to women and children impacted by domestic and family violence. This role focuses on practical, emotional, and advocacy-based support and requires a strong understanding of DFV risk, safety planning, and service systems. The position involves intensive client engagement and close collaboration with internal teams and external stakeholders. The position involves: Undertaking DFV risk assessments and safety planning Providing outreach-based case management support Court support related to Domestic Violence Orders and Family Law Supporting access to safe and sustainable accommodation Coordinating referrals and advocating across service systems Facilitating psychoeducational groups Maintaining accurate client documentation and data systems This role is not suitable for applicants without Domestic and Family Violence case management experience. Professional integrity, including privacy, confidentiality, and child-focused practice 5. Experience facilitating psychoeducational groups and understanding group facilitation principles 6. Ability to work collaboratively within a multidisciplinary team and with external stakeholders 7. Experience applying a case management framework, including documentation and use of client management systems 8.
