Respite Assistant Coordinator Hawthorn House – Albany Alzheimer’s WA is the leading dementia support organization in Western Australia, supporting people living with dementia, their families, and carers through advocacy, education, and specialist services. We are currently seeking a Respite Assistant Coordinator to join our team at Hawthorn House, our residential respite service in Albany. About the Role This is a full-time permanent position based at our Albany house. As the Respite Assistant Coordinator, you will be the backbone of our care team—providing essential administrative and operational support that enables Care Coordinators to focus on client care and leadership. You will support the coordination of respite services, maintain accurate client and service records, assist with compliance and quality requirements, and work closely with clinical and care staff to ensure safe, person-centred service delivery. Your work will directly contribute to improving the lives of people living with dementia and their families. This role is ideal for an experienced aged care professional ready to step into a coordination-focused position while remaining closely connected to quality service delivery. Key Responsibilities Assist with onboarding clients, including preparing accurate Agreements and Budgets Maintain up-to-date client records and documentation in CRM systems Support risk assessments, care planning, and service coordination Liaise with Registered Nurses to escalate clinical concerns and assist with non-clinical tasks Communicate effectively with clients, families, and Support Workers to ensure quality service delivery Document incidents, restrictive practices, and progress notes in line with policy and legislation Contribute to continuous improvement initiatives, audits, and compliance reporting Support team leadership, performance monitoring, and staff development What Success Looks Like in Your First 6 Months Confidently managing client onboarding, records, and service coordination Strong working relationships with Care Coordinators, RNs, Support Workers, and families Accurate, compliant documentation across CRM and reporting systems Active contribution to service improvement and team support Demonstrating calm, values-driven leadership in a dementia care environment. Essential Requirements Certificate III, IV, or Diploma in Community Services or a related discipline Experience in aged care, with dementia care highly desirable Strong knowledge of CHSP, HCP, and Aged Care Quality Standards Excellent organizational, communication, and time management skills A collaborative mindset and commitment to high-quality care Full working rights in Australia National Police Clearance (within 6 months) Current WA Driver’s License Why Join Alzheimer’s WA?
