The Role You are a contemporary Registered Nurse with a strong commitment to delivering high‑quality, person‑centred clinical care to older people living independently in the community. You are responsible for providing safe, evidence‑based nursing care to home care clients, supporting them to maintain health, wellbeing, independence and dignity in their own homes. You work collaboratively with clients, families, care workers and the broader multidisciplinary team, providing clinical oversight, education and coordination of care across community settings. This role will be based out of our Drummoyne location due to needs for this service of current clients, however travel to other areas of Sydney may be required in this role so a car and willingness to travel around the Sydney metro area will be essential. Your Key responsibilities: Build and maintain strong, trusting relationships with clients, families and carers in the community. Conduct comprehensive clinical assessments, including physical, cognitive, psychosocial and environmental assessments in clients’ homes. Plan, implement and evaluate nursing care, including wound management, medication management, chronic disease management, dementia care and palliative approaches as required. Develop, review and update person‑centred care plans in collaboration with clients, families and the care team, ensuring informed consent underpins all clinical decisions. Identify, escalate and respond to clinical risks, deterioration or changes in a client’s condition. Participate in case conferences, care reviews and multidisciplinary discussions to support coordinated care. Liaise with GPs, allied health professionals, hospitals and external service providers to ensure continuity of care. Provide education, guidance and delegation to care workers within scope of practice, ensuring safe and effective care delivery in the home. Complete accurate, timely and compliant clinical documentation in line with organisational requirements, Support at Home guidelines and accreditation standards. Contribute to clinical audits, incident management, feedback handling and continuous quality improvement activities. Essential experience: Registered Nurse Division 1 with current AHPRA registration and practising certificate. Demonstrated commitment to person‑centred care in community or aged care settings. Strong clinical assessment, care planning and documentation skills. Sound understanding of Home Care Packages, community aged care funding and the Aged Care Quality Standards. Experience working autonomously in community or home‑based settings. Well‑developed communication and interpersonal skills. Strong knowledge of safe work practices, including WHS in community environments. Intermediate computer literacy, including use of clinical and care management systems. Current driver’s licence and ability to travel to clients’ homes.
