Your Role as a Case Worker - Identified for our Housing and Support Service Link You will play an integral role in our newly created HSSL program providing culturally safe, trauma informed and holistic case management and supportive services to young parents (typically ages 16–24) who are experiencing homelessness, housing instability, or other barriers to independence. The role focuses on building trusting relationships, strengthening young people’s connection to culture and community, and supporting them to achieve their housing, wellbeing, and life goals. You’ll achieve this by: Working from a strengths-based and culturally grounded approach, recognising the resilience, knowledge, and cultural identity of young people and their families. Engaging and walking alongside young people through culturally safe processes ensuring case plans reflect the young person’s voice, cultural identity, and aspirations, and are developed in partnership with them, their families, and community. Supporting young people to develop confidence, independence, and practical living skills, while maintaining strong connections to culture, family, and community. Facilitating and supporting access to programs, support groups, and community networks that align with each young person’s goals and cultural needs. Providing regular updates and reporting to management and contributing to continuous learning and improvement in culturally safe practice. Actively contributing to program development and evaluation by embedding, community feedback, and cultural knowledge. What We’re Looking For Completed degree qualification in Human Services or another related field. Experience engaging with a diverse range of people using coaching and case management principles in a holistic, trauma informed and strengths-based framework. Experience in an intake, assessment, case management, advice, advocacy and referral role or similarly related skills and knowledge. Understanding of the social, emotional, and structural factors that place young people at risk of or experiencing homelessness, including family conflict, trauma, mental health concerns, substance use, financial hardship, and barriers to safe and stable housing.
