C Clinical Care Partner – Community Services ð Gorges Hall (On-site) ð° Salary: $85,000 – $110,000 super ⨠Salary Packaging: Up to $15,900 tax-free additional $2,650 per year (meals & entertainment) About the Role We are seeking an experienced Registered Nurse to join our Community Services Team , supporting older people to live safely and independently at home under the Support at Home Program . This role combines clinical care, care coordination, education, and advocacy , working across home care services, group social support programs, and seniors’ activities. You will deliver person-centred care with a strong focus on wellness, reablement, and independence .
