Job Description: Exciting opportunity to join our acute team Ongoing education and development opportunities Permanent / Full Time position based at Fitzroy No after hours or on-call roster About the Role An opportunity currently exists for a Grade 1 Social Worker to join our team in acute at St Vincent’s Hospital, Fitzroy. To fulfil this role, it is essential that you have current experience working in an inpatient hospital social work position post completion of your social work qualification. This could be in a sub-acute setting, however, experience in an acute setting will be highly regarded. To perform this role, you will: Use your skills to undertake multi-dimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate complex intervention and discharge plans Liaise with patients, their families and/or carers to co-ordinate, facilitate and communicate patient treatment and outcomes Actively participate in quality improvement and research activities Promote and develop positive relationships within the department, wider organisation and with community partners Classifications for Grade 1 Social Workers range from SC13 – SC16 ($74,172 - $88,872 per annum).
