We are currently seeking a Registered Nurse to join our team. As a Registered Nurse at Tamworth General Practice, you will play a pivotal role in delivering high-quality care and support to our diverse patient population. Your responsibilities will encompass but not limited to: • Conducting patient assessments, creating care plans, and health screenings with focus on GPMP, TCA, 715, and 75 health assessments. • Administering medications, vaccinations. • Perform chronic disease management, including monitoring patients with diabetes, hypertension, and other chronic conditions. • Maintain accurate patient records and assist with data collection for quality improvement initiatives. • Supporting GPs. Collaborating with administration and management to ensure efficient clinic operations and delivery of high-quality patient care. Requirements: • Registered Nurse qualification with current registration with AHPRA. • Experience as a Practice Nurse, preferably in a primary care or general practice setting but not essential • Experience in general practice and with Best Practice software is advantage but not essential. • Understanding of Medicare billing processes and experience in care plan reviews, and health assessments. • Commitment to providing compassionate, patient-centered care and contributing to the success of the practice.
