Job Description About the position: This 12‑month maximum term full-time role offers valuable experience within an established health service. As ITC Access and Care Coordinator, you will work within our Primary Health Care team to provide practical services and support to the community. Your responsibilities will include: Provide primary and secondary intervention services to improve health outcomes for Aboriginal and Torres Strait Islander clients; Coordinate referrals, access to services, data reporting, and supplementary services including visiting chronic disease specialists and allied health providers; Advocate for clients and assist in removing barriers to healthcare access through liaison with local Indigenous communities and groups; Advocate for clients and assist in removing barriers to healthcare access through liaison with local Indigenous communities and groups; Maintain effective relationships with health service providers and stakeholder networks to increase access; Confirm client eligibility and register clients in ITC program systems; Send program acceptance letters to GPs and welcome letters to clients; Maintain and update referrals, recall systems, and administrative processes to support chronic disease care; Provide recommendations to the Clinical Practice Manager to improve efficiency.
