About the Role: We are seeking a compassionate and proactive Community Worker to provide support to people with disabilities and their families. This role involves assessing individual needs, developing tailored support plans, and working collaboratively with a range of services to deliver positive outcomes. You'll be part of a team committed to advocacy, empowerment, and social inclusion. Key Responsibilities: · Work directly with clients to identify individual needs and deliver person-centred support, education, and capacity-building programs that promote independence and wellbeing; · Assist clients to develop independent living skills, social participation, and community engagement, in accordance with individual support plans and NDIS Core and Capacity Building supports; · Conduct interviews, client intakes, and ongoing needs and progress assessments, contributing to goal planning and service coordination in line with organisational policies; · Provide information, education, and support to clients and families to enhance wellbeing, self-reliance, and access to appropriate community and government services; · Maintain accurate client records, case notes, and reports, ensuring compliance with NDIS Practice Standards and relevant legislative and organisational requirements; · Refer clients to appropriate community services, allied health providers, and external agencies when additional or specialised support is required; · Liaise with community organisations, government departments, and service providers to coordinate services and advocate for inclusive, accessible, and culturally appropriate supports; · Identify service gaps and contribute to the development and improvement of community programs and support networks in areas such as disability support, housing, employment, education, and social inclusion; · Provide support to individuals with disabilities and other vulnerable groups in home-based, group, and community settings, promoting safety, dignity, choice, and independence.
