This is a genuine opportunity to join at a pivotal time and help shape what the “Older Adult Drive” Benefits Adults-only caseload Contracting OR permanent options (full-time or part-time pathways available) Travel support: company vehicles available mileage reimbursement (up to $0.99/km) New office space from 1 April: larger, modern setup with private parking A leadership team committed to reducing admin load and improving systems (intake/process improvements underway) Real career runway as the service expands into Perth metro Work that matters: helping people move safely from hospital back into the community Seniors encouraged to apply to become team leaders immediatly About the role: you’ll work across a mix of: Hospital transition support: liaising with ward staff, completing assessments, and contributing to discharge plans so clients can return home safely and confidently Residential aged care facilities: regular onsite clinical work and collaboration with facility teams In-home services for older Australians: community visits including Home Care Package clients and some NDIS (where relevant) Speech Pathologists may be involved in Dysphagia assessment/management, mealtime supports, staff training Communication and functional participation supports for adults/older adults Collaboration with hospitals and aged care teams to reduce re-admissions and improve quality of life Occupational Therapists may be involved in Functional assessments, ADL support, discharge planning input Equipment recommendations, home safety, falls risk considerations Liaising with families/support networks and facility staff to implement practical, client-centred plans Support available Structured onboarding (3 months) with regular check-ins and goal setting Access to experienced clinicians (including senior clinicians as the team grows) A leadership style that is visible, approachable, and hands-on Team culture: This is a team in rebuild mode — and that’s the exciting part.
