Job Description We are looking for an experienced Community Based Clinical Care Manager - Registered Nurse to join our client's team on a Permanent Full Time basis. Our client is a not-for-profit specialising in supporting frail aged people in metro Western Sydney, particularly from diverse multicultural backgrounds. About the role As a Community Based Clinical Care Manager - Registered Nurse, your will be working as part of our Home Care Packages team and will be responsible for: Clinical care needs assessment and reviews Developing and reviewing clinical care plans Nursing care and support for HCP Participants Liaise and coordinate with multidisciplinary teams, including care coordinators and support workers Leading our IPC management system, including providing training and support to staff For the ideal and experienced candidate - there would also be a clinical governance aspect to this role. You will contribute to our Clinical Governance team, working towards ensuring that assessed and delivered clinical care plans and strategies achieve optimum outcomes, standards, flexibility, and responsiveness for participants within legislative and regulatory requirements of the program, strengthening aged care standards and the available resources. Requirements: Minimum 2 years Registered Nurse experience in Aged Care, NDIS or Home Care environment Bachelor of Nursing and active AHPRA registration Sound clinical knowledge and Care planning experience Competent in male and female catheterisation and chronic wound care management A genuine passion for helping the elderly community remain independent Outcomes-focused and solution-orientated with high level of ability to take initiative.
