Role details The Specialist Palliative Care Services at the Mackay Base Hospital is seeking applications for a Clinical Nurse Consultant (Service Lead) to join their team on a permanent basis. Your opportunity The Clinical Nurse Consultant (Service Lead) provides senior clinical and operational leadership to ensure safe, high quality and patient centred care within the Specialist Palliative Care Service. The role delivers expert clinical leadership, advanced nursing practice in clinical consultation while leading workforce capability and performance across the team. The Clinical Nurse Consultant (Service Lead) is accountable for day-to-day service coordination, clinical governance and risk management in alignment with organisation requirements. The position drives service improvement initiatives using data, and evidence and consumer feedback. The Clinical Nurse Consultant contributes to strategic planning, workforce sustainability and continuous improvement to meet current and future demands. The Clinical Nurse Consultant is an experienced nurse with expert clinical knowledge and an in-depth understanding of Palliative Care, symptom management, and end of life care planning. The Clinical Nurse Consultant employs advanced clinical and professional practice capabilities to deliver a positive patient-centred health experience for individuals and their support persons and works collaboratively with members of the care team and multiple stakeholders to implement sustainable models of care. About the Team The Mackay HHS Specialist Palliative Care Service aims to improve the quality of life for patients and their families facing the problems associated with life-limiting illness. This can be achieved through the prevention and relief of suffering by means of early identification and treatment of pain and other problems, in the physical, psychological and spiritual domains The Mackay HHS Specialist Palliative Care Service provides palliative care support to inpatients at Mackay HHS, outpatient clinics and a community program to assist patients and carers in their own home within a 45km radius of Mackay. Specialist palliative care support in the Isaac, Bowen and Whitsunday regions, are provided by the Specialist Palliative Care Rural Telehealth Service team based at the Townsville Hospital and Health Service. A multidisciplinary team working within specialist palliative care provides holistic, co-ordinated care to patients and their carers across the care continuum, with close collaboration with other health professionals.
