Troocoo are seeking an experienced Care Planner (Registered Nurse) to join a residential aged care service. This role is ideal for an RN who is passionate about person-centred care and enjoys ensuring residents receive high-quality, individualised support. You will be responsible for care planning, assessments, and ongoing reviews while working closely with residents, families, and internal teams to ensure care aligns with quality and funding requirements.
