As a Live Easy Team Leader, you play a critical leadership role in ensuring high-quality, person-centred care is delivered consistently to our customers. You are responsible for supervising, training and supporting Live Easy Buddies, while also maintaining a direct caseload and delivering care and care management services to customers. You will lead by example, providing hands-on support, ensuring effective rostering and continuity of care, supporting onboarding of both customers and staff, and contributing to quality, compliance and continuous improvement activities. Your role bridges frontline service delivery and operational leadership, ensuring Live Easy by Scalabrini’s values, standards and customer experience expectations are upheld. Some of the Key Tasks of the role include: Provide day-to-day supervision, guidance and support to Live Easy Buddies. Act as a mentor and role model, promoting best practice in person-centred care and service coordination. Support the onboarding and induction of new Live Easy Buddies, including orientation, shadowing and competency development. Identify training needs and support Buddies’ ongoing learning, professional development and performance growth. Monitor Buddy performance, provide regular feedback, and participate in performance reviews and corrective action processes where required. Be responsible for ensuring effective rostering and scheduling of customer visits delivered by Live Easy Buddies. Organise coverage and handover arrangements when Buddies are on leave, unwell or unavailable to ensure continuity of care Deliver direct in-home care and support to allocated customers, consistent with the Live Easy Buddy role. Manage an allocated customer caseload, including assessments, care planning, implementation, review and documentation. Act as the main point of contact for allocated customers, advocating for their needs and preferences. Match and allocate new customers to suitable Live Easy Buddies based on needs, preferences, skills and availability. Undertake quality activities including audits, file reviews and documentation checks. Ensure care delivery, documentation and practices align with legislation, funding guidelines and strengthened quality standards. You will ideally have: A Certificate III in Individual Support (ageing and disability) Demonstrated experience in home care, aged care, disability or community services Experience in a senior, supervisory or team-leading role (desirable) Strong leadership, coaching and mentoring skills Excellent organisational, rostering and time-management skills High-level communication and stakeholder engagement skills Sound understanding of home care funding, care coordination and quality standards Please Note: You will need a NSW drivers licence and access to your own vehicle as this role will involved travel around Sydney.
