The Role As a Care Planner, you will play a vital role in promoting safe, high-quality care and ensuring accurate funding outcomes. Working closely with the Care Manager and Care Team, you will ensure care plans are current, meaningful, and truly reflect each resident’s needs, goals, and preferences. Key Responsibilities Complete interim assessments and care plans on admission Conduct quality care evaluations Develop care plans in genuine partnership with residents and families Liaise with revenue and classification teams to ensure accurate AN-ACC classification Ensure care delivery aligns with Aged Care Quality Standards Champion a resident-first, person-centred approach About You You are a dedicated and compassionate Registered Nurse who thrives in a collaborative environment and is passionate about quality care. You will bring: Current AHPRA registration as a Registered Nurse Minimum 2 years’ experience in a care management or similar role within residential aged care Strong written and verbal communication skills A commitment to person-centred care Current NDIS Worker Screening Check (or willingness to obtain) Why Yackandandah?
