We are currently seeking two Clinical Care Partners to join our Support at Home team and make a meaningful difference in the lives of our clients across Tasmania. These roles can be based in Hobart, Launceston, Devonport or Burnie , offering flexibility and the opportunity to work close to home while supporting clients across the state. This role requires current AHPRA registration as a Registered Nurse. About the role As a Clinical Care Partner, you will deliver high-quality, person-centred and culturally safe clinical care management services to older people living at home. Working in alignment with the Support at Home program and our clinical governance framework, you will play a vital role in ensuring clients maintain independence, wellbeing, dignity and safety in ways that reflect their culture, identity and preferences. There may be a requirement for some travel within Tasmania to support clients, teams and service delivery needs. You will: Undertake comprehensive clinical assessments, care planning and service coordination. Provide evidence-based clinical risk assessment, education and guidance for clients with complex needs and chronic health conditions. Offer clinical oversight and support to non-clinical Care Partners, ensuring early identification and management of clinical risks and deterioration. Ensure services are delivered in line with the Strengthened Aged Care Quality Standards. About you You are a dedicated and compassionate health professional who thrives on building meaningful connections with clients and colleagues. With a strong clinical background, you are confident in providing clinical oversight and coaching while maintaining the highest standards of care. You will bring: Current registration with AHPRA as a Registered Nurse (or equivalent clinical qualification). Experience in community care, aged care or chronic disease management. Strong clinical assessment and care planning skills. The ability to work collaboratively across multidisciplinary teams. A strong commitment to person-centred and culturally safe care.
