We currently are seeking an experienced Disability Support Worker (Disabled Carer) and a Disabilities Services Officer to join our Tiwi team in full time roles, with work also in remote communities in Nhulunbuy, Galiwin'ku. Disability Support Worker (Disabled Carer): The Support Worker (SIL/Respite House) is responsible for providing high-quality care and support to participants in Supported Independent Living (SIL) and respite care settings. This role involves assisting participants with daily living activities, promoting their independence, and ensuring their safety and well-being. Applicants for this role must have at least 3 years' experience as a Disability Support Worker and hold at least a Certificate III in disability services. Responsibilities include: Assist participants with personal care tasks such as bathing, dressing, grooming, and toileting. Support participants with meal preparation, feeding, and medication administration. Assist with household tasks, including cleaning, laundry, and shopping. Monitor and support participants' health and well-being, reporting any concerns to the appropriate supervisor or healthcare professional. Encourage and facilitate participation in recreational and social activities to promote mental and physical health. Support participants in developing and maintaining independent living skills, such as budgeting, cooking, and using public transport. Empower participants to make choices and decisions about their own lives. Ensure the safety and security of participants by following established protocols and procedures. Identify and mitigate risks, ensuring a safe living environment for all participants. Maintain accurate and up-to-date records of participant interactions, progress, and any incidents. Prepare and submit reports as required by the organization. Communicate effectively with participants, their families, and other team members. Collaborate with healthcare professionals, service providers, and other stakeholders to ensure comprehensive care. Respond to and manage emergencies and crises involving participants. Implement appropriate interventions and report incidents according to organizational policies. Ensure all activities and services comply with NDIS guidelines and organizational policies. Stay informed about changes in policies and best practices in disability support. Disabilities Services Officer: The Disabilities Services Officer is responsible for assisting NDIS participants in understanding and implementing their NDIS plans. This role involves working closely with participants to identify their goals, connect them with appropriate services, and ensure they receive the necessary support to achieve their aspirations. Applicants must have at least 3 years' experience in a similar Disabilities Services Officer role. Responsibilities include: Assist participants in understanding and implementing their NDIS plans. Work collaboratively with participants to identify their goals, needs, and aspirations. Develop and maintain strong relationships with participants, ensuring a person-centered approach. Coordinate the implementation of all supports in the participant's NDIS plan, including informal, mainstream, and community supports. Monitor and review participant plans, ensuring supports are effective and meet the participant's goals. Connect participants with appropriate service providers and community resources. Facilitate communication and collaboration between participants, their families, and service providers. Ensure services are delivered in a timely and efficient manner. Empower participants to exercise choice and control in their support and services. Provide guidance and support to participants to build their capacity to manage their own supports. Maintain accurate and up-to-date records of participant interactions, goals, and progress. Prepare and submit reports as required by the NDIS and the organization. Ensure all documentation complies with NDIS standards and organizational policies. Respond to and manage crises and emergencies involving participants. Develop and implement risk management strategies to ensure participant safety and well-being. Ensure all activities and services comply with NDIS guidelines and regulations. Stay informed about changes in NDIS policies and practices and ensure implementation within the organization. Build and maintain relationships with external service providers, community organizations, and other stakeholders. Advocate on behalf of participants to ensure they receive the necessary supports and services. Participate in professional development and training to enhance skills and knowledge. Assist participants in managing their NDIS budgets and resources effectively. Monitor spending and ensure services are delivered within budget constraints.
