With a booming, culturally rich and diverse community, and less than an hourâs drive from the CBD (still within the bounds of Myki and ride-sharing), Melbourneâs North is fast becoming the place to be.   Northern Health are seeking applications from motivated and suitably experienced Physiotherapist to join the Transition Care Physiotherapy team at Northern Health.  Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourneâs rapidly growing outer north.  Services are provided through our six main campuses: Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.  About the Role: The Grade 2 Physiotherapist will have a leading role in ensuring excellence in client care and education of staff and students in the physiotherapy stream. The role encompasses the delivery of Physiotherapy services in the Transition Care Program.  This role is responsible for providing leadership and ensure high level, evidence-based physiotherapy care to our patients within the Transition Care Program (TCP). The role includes the organisation, implementation, development and supervision of physiotherapy care and the promotion of best practice. The caseload for this position consists of treating bed based and home based clients with a range of general conditions. The Northern Health Transition Care Program consist of: Bed Based TCP, Home based TCP and GEM at Resi. Care may be delivered in community, hospital and residential care settings.  This role is a permanent, full time (80 hours per fortnight) position. Part time/job share applications will also be considered.  To be successful in this role you will have: A background which includes significant clinical experience resulting in high level of skill in gerontology physiotherapy Experience managing patients in the subacute, residential aged care or community setting Demonstrated highly developed interpersonal and communication skills Experience and/or interest in the management and education of undergraduate physiotherapy students and Allied Health Assistants Demonstrated capability in service innovation and development Demonstrates leadership skills and initiative in developing and promoting excellence in clinical practice  Next Steps . . .
