Your opportunity: The Aged Care Assessor within the Wide Bay Aged Care Assessment Team is required to work within a multidisciplinary team and provide clinical assessment services to clients through the use of high level clinical skills, knowledge and experience and highly effective communication. The position works within an integrated hospital/community Aged Care Service across the Service Area in order to assess clients to access appropriate support services and/or residential care. This position reports professionally to the relevant Professional Lead and operationally to the Aged Care Assessment Team Leader, Integrated Care, Wide Bay Hospital and Health Service.
