The Role As a Care Champion you will not have direct reports, but you would indirectly lead other Personal Care workers within your home. You may also be required to help deliver care to residence to assist other personal care workers. You would be an important member of our Home Leadership team, playing a vital role in delivering the best care to our residents and champion Bolton Clarke’s purpose in helping people live a life of fulfillment each and every day. A frontline advocate for a group of residents. You will take ownership of residents’ wellbeing and care by maintaining a schedule of check-ins to ensure all their needs are met. Understand each resident you care for as an individual with unique needs and encourage regular feedback (formal and informal) Proactively problem solve and attend to gaps in resident needs through collaboration and teamwork. Communicate and work with the care team to ensure consistent and high-quality resident care and support. Demonstrate the Bolton Clarke Values-in-practice to foster a culture of kindness, curiosity and great care To be the day-to-day relationship link for families, keeping them up to date with the care and wellbeing needs of the resident. Build strong and inclusive relationships by communicating effectively with residents, families, and staff team members. About You To be successful, you'll need: Certificate III in Aged Care, Individual Support or equivalent Previous experience as PCA (3 years plus essential) Commitment to work various hours with flexibility to pick up additional shifts Ability to communicate and build rapport with residents, families and team members The Successful applicants must complete a National Police Check & Free seasonal flu vaccination and possibly complete a NDIS Worker Screen.
