Job Description The Hive Team support program participants to maintain or regain their independence, continue living safely in their home and actively participate in their community.to The Hive is funded by the Queensland Community Support Scheme (QCSS). The intent of the Queensland Community Support Scheme (QCSS) is to provide low intensity basic care and supports to assist eligible people to increase their connection to their local community and live independently. The goal of QCSS is to empower participants to engage with their local community, increase their capacity to live independently and, as appropriate, facilitate access to the broader service system to further support vulnerable participants. There are two service types in QCSS, Direct Care and Support and Community Connection Support We are seeking a dynamic and experienced Support and Advocacy Worker , you will provide direct care to individuals to live as independently and safely as possible in their home as well as provide Community Connection Support to increase opportunities for participation in, and access to community of choice. This is a casual employment opportunity with conditions and salary at Level 3 of the SCHADS Award. Desired Skills and Experience Key Responsibilities Assist individuals with domestic activities, such as assisting with meal plans, meal preparation, cleaning, washing and general household responsibilities. Assist with household correspondence including making appointments, placing reminder calls, managing paperwork and bills, and using technology such as internet and mobile applications. Provide support to individuals to access essential services, such as the bank, post office, grocery shopping. Provide support to individuals to access and engage (or re-engage) with primary health services (e.g., general practitioner, community mental health team), including assistance to make and maintain appointments. Essential Criteria: Demonstrate sensitivity to individuals' needs and report changes in supported person’s wellbeing Demonstrated commitment to upholding child and vulnerable adult safeguarding principles. Demonstrated understanding of the impact of mental health, disability and drug and alcohol on a person’s lifestyle, choices, and knowledge of providing a service appropriate to culture, age, sexual orientation, disability, and socio-economic status of individuals. Demonstrated ability to use effective communication strategies and processes involving well developed interpersonal skills, an ability to advocate effectively, documentation of assessments and individual advocacy and support plans. Experience providing holistic social inclusion activities and events to people who may have a reduced awareness of their personal care and social support needs. Desirable: Tertiary studies in human services or equivalent. Experience in continuous improvement processes.
