Supportive, patient centred environment Strong focus on quality improvement and innovation Collaborative links with Blacktown and Mt Druitt Hospitals Access to specialist teams including endocrinology and allied health Commitment to professional growth and development What You'll Be Doing In this key role, you will coordinate and evolve the Osteoporotic Refracture Prevention Service, ensuring patients receive timely, evidence-based care and long term support. You will enhance access to assessment, treatment and education while leading improvement initiatives across the district. Coordinate and deliver comprehensive bone health assessments and education Manage the ORP clinic with the endocrinologist and multidisciplinary team Support patient self-management and facilitate seamless access to services Lead quality improvement, data collection and evaluation of the service Provide expert advice, clinical pathways and education to staff and stakeholders How To Apply Apply online by submitting your resume and addressing the criteria listed below by providing examples that demonstrate your ability before the closing date. Essential Criteria Registration:Current AHPRA Registration: Allied Health Professional Selection Criteria Relevant Allied Health qualifications with significant post graduate clinical experience involving populations with musculoskeletal conditions and/or chronic diseases. Demonstrated interest in musculoskeletal health care including extensive knowledge of the contemporary issues around osteoporosis and its management. Well-developed understanding of the concepts of chronic care and its application with people and communities where chronic care is required. Demonstrated experience in planning and coordinating multi-disciplinary care and the capacity to work cooperatively and effectively with medical, allied health professionals and consumers with the ability to provide expert clinical education and training. Demonstrated highly developed interpersonal, written and electronic communication skills and the ability to build and maintain relationships. Proven ability to take initiative, make decisions, and work both autonomously and in a close team environment. Proficiency in Information Technology such as Microsoft applications, email, clinical software packages and an ability to manage and maintain database information with accuracy, proficiency and attention to detail.
