Liaise with medical, allied health, pharmacies, service providers and equipment providers to ensure a holistic, comprehensive delivery of care. Document and report care interventions, observations, changes in needs and consumer feedback in a accurate, objective and efficient manner. Coordinate staff as required on a shift-by-shift basis Assist in the review and dissemination of clinical policies, procedures and assessment tools under the direction of the Clinical Nurse Manager. Provide clinical advice and education to persona l care staff to ensure appropriate care.
