We are seeking a highly skilled and autonomous clinician to join our team, providing advanced, evidence-based care to consumers within Hospital in the Home (HiTH), Residential Aged Care Facilities (RACFs), Geriatric Emergency Management Rehab-Hospital in the Home (GEMR-HiTH), Interim Care, Residential Aged Care Support Services (RASS), and the Mackay Watchhouse to improve access to health care and reduce the demand on hospitals by improving flow and promoting the right care in the right time for the right outcome. About the RoleIn this pivotal role, you will utilise your expanded clinical capabilities to deliver high-level, patient-centred care across multiple settings. You will work autonomously, applying advanced assessment and management skills to ensure safe, effective, and timely healthcare delivery. Your responsibilities will include: Delivering advanced clinical assessment, diagnosis, and management for acute patients within HiTH, RACFs, and custodial environments. Providing inpatient hospital-standard acute care substitution within RACFs for eligible consumers. Reviewing residents in RACFs at the request of GEDI, CNC, or medical staff to prevent unnecessary Emergency Department presentations. Conducting clinical reviews for individuals in custody within the Mackay Watchhouse at the request of QPS or medical staff to prevent unnecessary Emergency Department presentations. Utilising a case management model to coordinate comprehensive care for HiTH, GEMR-HiTH, Interim Care, and RaSS patients. Exercising full Nurse Practitioner scope, including prescribing medications, ordering diagnostics, and referring directly to other healthcare professionals. Supporting safe alternatives to hospitalisation by increasing access to high-quality clinical care in community, aged care, and custodial settings. Demonstrating a special interest and expertise in the care of older persons. About YouYou will be a confident, autonomous practitioner with: Current registration as a Nurse Practitioner with AHPRA. Advanced clinical decision-making skills and demonstrated experience in acute or aged care settings. A passion for delivering safe, high-quality care to older persons and vulnerable populations.
