We’re expanding and are looking for a dedicated EN to join our Access Aged Care team across the Central Coast. This is a big job and would suit a nurse who has Aged Care experience and is highly organised with strong critical thinking skills. What You’ll Do Work alongside RACF care teams as the on site Access Aged Care nurse. Facilitate GP and Specialist rounds (both in-person and telehealth) to ensure smooth, timely care. Prepare, organise, and triage referrals to maximise efficiency and outcomes. Conduct bedside assessments and clinical reviews. Act as the communication bridge between RACFs, GPs, and families. Deliver excellent customer service, with empathy and professionalism at every interaction Build strong relationships with residents, families and facility partners. Aged Care experience (desirable) Strong organisational, problem-solving and communication skills. Confidence with IT and documenting resident needs. A driver’s licence and reliable vehicle.
