Day-to-Day Responsibilities: - Providing support to our clients to improve their independence by assisting with living skills, transport, social activities, medication, and personal care. - Keeping customer records up to date using online systems. - Developing and maintaining professional relationships with people, families, caregivers, and key stakeholders. - Assisting in the development or implementation of care plans. You are invested in providing the highest quality in disability services and are competent in working with people with a disability. Requirements: - Appropriate certificate level qualification in Disability or Community Services/Individual Support or equivalent relevant experience in the human services industry. - Valid NSW Drivers C class license, with the ability to drive vehicles in license class, including large cars and vans. - Must be fit to undertake the physical requirements of customer support, including the capacity to lift 10 kilograms unassisted. - Intermediate skills in Microsoft Office and other common computer systems. Desirable: - Experience with disabilities and mental health conditions - Advanced knowledge of services and programs under NDIS. - Understanding of Person-Centered Care Practices.
