Providing Health Assessment, diagnosis and treatment services to patients at the Practice, Home and Nursing home visits when required. Ordering diagnostic tests as needed, checking and following-up of results in a timely manner Provide appropriate Health promotions and preventative Health care advise to all patients attending the Practice Provide management of long term conditions in line with National guidelines Referring patients appropriately to other providers when necessary Documenting all care provided and education/Information given to patients within their medical health record, as per professional and practice Standards/protocols. Clearing in-boxes daily and delegating this task if absent
