Your key responsibilities: Deliver high-quality clinical case coordination, assessments, and treatment interventions within a strong clinical governance framework. Work collaboratively with a multidisciplinary team to ensure seamless, person-centred care for clients. Improve access to primary health services for CALD communities by addressing local service gaps across Northern Sydney. Identify health needs and develop targeted intervention plans that make a real difference in client outcomes. Provide consultations, health education, and timely referrals to primary and allied health services both onsite and in the community Build strong partnerships with community providers, allied health professionals, and GPs to deliver coordinated, continuous care. Lead and participate in community events and initiatives that promote health awareness and strengthen connections within CALD communities.
