About the opportunity The position is responsible for ensuring the implementation of individual assessment, diagnosis, treatment, follow up, multidisciplinary consultation, liaison with referring agents, discharge planning and evaluation of care using appropriate outcome and data measurement systems.The Community Care Team provides a range of services to consumers in the short, medium and long term. These include: Individual treatment programs, family interventions, individual and group therapy, psychology services, psychiatric outpatient clinics and case management. The Community Mental Health Service provides community-based assessment and treatment using case management in the context of a multi-disciplinary team, for people with moderate to severe mental health problems.
