We have an opportunity for a Clinical Care Coordinator responsible for the delivery of resident focussed clinical care services including planning, review, and assessment of all aspects of clinical care. Reporting directly to the Resident Services Manager, this full time position (Monday to Friday 0700-1506) will ensure clinical care delivery enhances the wellbeing, dignity, and quality of life for residents. Key Responsibilities: Ensure comprehensive clinical and specialised nursing care is provided, documented, and tailored to each resident's needs. Prioritise residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks. Comply with all reporting requirements including incident and SIRS/mandatory reporting. Oversee and conduct all scheduled AN-ACC re-appraisals, including mandatory and voluntary assessments, as required. Encourage and support Registered Nurses (RNs) and Enrolled Nurses (ENs) to be accountable for daily case reporting, care planning, and addressing patient concerns promptly. Conduct routine clinical assessments to promptly address any identified gaps in care. Provide guidance, advice, assistance and support to all clinical staff in a manner that is empowering, encouraging and driven by best practice clinical principles and procedures. Experience and Skills: Current AHPRA Registered Nurse (DIV 1) registration with no conditions. Dedication to resident care and support. Excellent clinical knowledge regarding medical conditions, palliative care and care of people living with dementia, including managing challenging behaviours. Advanced clinical skills and experience in managing outbreaks and emergency situations. Proven ability to motivate and lead staff, promoting a positive work environment. Highly developed interpersonal and written skills, and the ability to effectively communicate with residents, resident representatives and other health professionals. Strong planning, time management and organisational skills. Ability to complete reporting and conduct trend analysis and observations to inform best practice care. A good understanding of the Strengthened Aged Care Act and its principles, Aged Care Standards, and AN-ACC funding, and palliative care funding.
