Montefiore has a wonderful opportunity for an experienced Nurse Practitioner to join the team. This exciting position has scope to work across our organisation as well as future scope to provide NP services to our Independent Living Units. Reporting to the General Manager Residential Care, you will provide expert advanced assessment, advice and clinical leadership in collaboration with the interdisciplinary team, Local Medical Officers and external stakeholders. About you: You will have strong clinical capabilities and knowledge to provide leadership, education and mentoring to nursing staff to improve clinical care outcomes for consumers. You will enjoy a diverse caseload with a strong focus on hospital avoidance, antimicrobial stewardship, palliation, falls analysis, diabetes management, medication management and behaviour support. You will be comfortable chairing committee meetings and will be committed to evidenced-based practice. You will demonstrate excellent time management and organisational skills and have a true passion for continuous quality improvement and for the aged care sector. To be considered for the role, you will possess the following: Essential Criteria: Current registration as a Registered Nurse with AHPRA with endorsement as a Nurse Practitioner Minimum of 3 years of Advance Nursing Practice with at least 5 years’ experienced as an RN Demonstrated advanced clinical skills in nursing older adults, acute and chronic diseases Demonstrated high level of interpersonal, communication and negotiation skills to effectively engage and collaborate with stakeholders to achieve optimal outcomes through critical analysis, problem solving and accurate decision making Demonstrated evidence of the use of critical and reflective thinking, complex reasoning, and analysis to guide assessments, clinical judgements and decisions whilst managing complete episodes of patient care Demonstrated ability to work both as an autonomous practitioner and to work collaboratively to provide quality care Demonstrate clinical leadership influencing and progressing evidence based clinical practises and procedures Computer skills in Microsoft and Client Information Management systems (e.g. Electronic Medication Management System, consumer records) Current NSW drivers’ licence and own transport (as travel across all Montefiore campuses is required) Desirable Criteria: Prior experience working within a residential aged care setting, with a contemporary person-centred approach to care Experience working on projects Interest and experience in participating/initiating and or facilitating high quality research About Us: Montefiore is an award-winning, Sydney-based aged care provider, with a proud 135-year history as a non-profit organisation founded on Jewish values.
