In this role, you will be the essential link between NDIS participants and the services they need. As a Coordinator of Supports you will collaborate with people with disability, their families and support networks to ensure they receive the right services and support that meets their individual needs. Responsibilities include (but are not limited to): Working with Participants to achieve their NDIS goals, build their capacity and independence. Linking participants to relevant service providers and coordinating services on behalf of Participants Working within individual NDIS funding budgets while continually tracking and monitoring billable hours. Have well developed interpersonal skills with the ability to build positive working relationships with internal / external stakeholders. Planning and managing individual workload to ensure the delivery of quality services in conjunction with NDIA and organisational reporting requirements Confident in meeting KPIs including billable hours Complying with all relevant legislation and codes of conduct What’s on offer?
