You’ll conduct intake and triage assessments, provide referrals, and help connect people to the right support by ensuring that every client feels heard, understood, and cared for from the very first contac t . The role’s key accountabilities are : Provide all individuals who contact the WMQ Mental Health intake team (primarily by phone and email) with a positive experience and are serviced appropriately Undertake assessments of customers utilising Initial Assessment and Referral Decision Support Tool to determine most appropriate program for their needs, risk assessment including suicide risk screen and initial outcome measurement tool collection Allocate referral to services which meet the clients’ identified needs, providing prompt response times, and managing demand and caseloads accordingly Where customers are ineligible or at risk, provide warm referrals to other more suitable supports in the stepped care continuum Create and maintain accurate records of intake, assessment, referrals, feedback and any follow-up actions Listen to and understand the needs of customers and use problem solving skills to work with them to identify any barriers to accessing therapy Work collaboratively as part of Mental Health Services team to ensure smooth client transitions between services when Contribute to a database of service providers, understanding their target client groups, capacity, and provide warm referrals accordingly in line with KPI timelines Prepare and distribute resource and information packs (electronic and hard copy) to customers and service providers Facilitate and build effective and positive relationships with all internal and external stakeholders Undertake general administration and reporting tasks as required and contribute to continuous improvement of the services offered Ensure all communication and interaction with customers is respectful, empathetic and reflects the WMQ values Maintain a high level of discretion and confidentiality through all interactions Record and action any complaints, compliments and feedback received by the program, escalating to the Program Manager where r equired Adhere to National Mental Health Standards and other significant organisational quality procedures What We’re Looking For Tertiary level qualifications in social work, mental health and/or other relevant discipline Demonstrated experience in working in mental health and in-depth understanding of mental health issues including best treatment options and relevant support services Demonstrated successful experience in an intake and assessment role within a multidisciplinary mental health setting or similar Demonstrated understanding of the issues affecting a wide and diverse variety of community members, particularly those with mental health issues Demonstrated experience in conducting mental health screening and risk assessment A broad knowledge of the mental health and community supports on offer in the Brisbane South and Gold Coast regions Excellent customer service skills including the ability to respect and develop rapport and empathy with the customer and referrer Sound ability to clarify customer queries, problem solve and utilise initiative to follow up customer contacts Ability to liaise and form effective partnerships with internal and external stakeholders Demonstrated ability to positively work as a senior member of a small team with minimal supervision and in an environment requiring flexibility Excellent organisational skills and ability to effectively and efficiently manage competing tasks Essential Requirements Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy) Relevant probity checks required by legislation and WMQ Limited policy.
