A unique and career-defining new role within the industry, our Care Champions will have a dedicated focus on leading and providing person-centred care for our residents. Ideally suited to passionate individuals with a Certificate III (Aged Care), you will work across a group of residents and their families to ensure they experience exceptional resident care. As a Care Champion you will not have direct reports, but you would indirectly lead other Personal Care workers within your home. You may also be required to help deliver care to residence to assist other personal care workers. You would be an important member of our Home Leadership team, playing a vital role in delivering the best care to our residents and champion Bolton Clarke’s purpose in helping people live a life of fulfilment each and every day. About The Opportunity A frontline advocate for a group of residents. You will take ownership of residents’ wellbeing and care by maintaining a schedule of check-ins to ensure all their needs are met. Understand each resident you care for as an individual with unique needs and encourage regular feedback (formal and informal) Proactively problem solve and attend to gaps in resident needs through collaboration and teamwork. Communicate and work with the care team to ensure consistent and high-quality resident care and support. Demonstrate the Bolton Clarke Values-in-practice to foster a culture of kindness, curiosity and great care To be the day-to-day relationship link for families, keeping them up to date with the care and wellbeing needs of the resident. Build strong and inclusive relationships by communicating effectively with residents, families, and staff team members.
