Position Title: Chronic Disease Nurse Classification: Community Health Nurse CN4 (ZJ1) Salary range per hour: $55.97 superannuation generous salary packaging option Employment Type: Ongoing part-time 32 to 48 hours per fortnight Award: Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028 Role description The Chronic Disease Nurse will work closely with other Chronic Disease practitioners including Nurse Practitioners, Diabetes Education and Cardiopulmonary Rehabilitation practitioners, to ensure comprehensive support is provided to enable a holistic approach to care. This role will provide an initial assessment on referral and assist clients to navigate through their chronic disease management journey. This role is designed to ensure that clients are supported to achieve their best outcomes. This is inclusive of care coordination, advocacy and collaboration across health services, ensuring all aspects of client health and wellbeing are considered. About you – Key Selection Criteria Registration with the Australian Health Practitioner Regulation Agency as a Registered Nurse in the State of Victoria and holding a current practicing certificate Demonstrated experience of providing chronic disease education in a community health setting Demonstrated commitment to, and experience in, applying the principles of the social model of health, health promotion and community development Demonstrated awareness of current issues related to health promotion and primary health care in a rural setting Demonstrated ability to participate in a collaborative, multidisciplinary team environment to develop, implement and evaluate activities Demonstrated high level of inter-personal skills with the ability to effectively liaise with patients, clients, the community, service providers and other agencies Demonstrated effective verbal and written communication skills and the ability to write program proposals, program evaluations and reports Please refer to the relevant attached position description and address the key selection criteria on your cover letter when applying.
