About the role Relationships Australia Queensland (RAQ) is seeking a compassionate and proactive practitioner to join our Senior Social Connection Program (SSCP). This vital program supports older people to strengthen their community connections, enhance wellbeing, and reduce social isolation. In this rewarding role, you’ll deliver person-centred, evidence-based support to empower older individuals to feel safe, connected, and confident. You’ll provide information, advice, referrals, and practical support — helping older Queenslanders access the right services and maintain meaningful social engagement. Additionally, you will: Provide responsive intake, information, and referral services to older people, their families, and carers. Conduct comprehensive assessments to identify supports that promote safety, connection, and wellbeing. Deliver tailored interventions and practical supports that address each client’s unique needs. Build partnerships with community organisations and services to enhance collaboration and access. Facilitate community engagement, education, and group activities to reduce social isolation. Maintain accurate documentation and ensure compliance with all organisational and legislative requirements. About You You are an empathetic and skilled professional who is passionate about improving the wellbeing and social connection of older Australians. You bring a strengths-based approach, excellent communication skills, and a deep understanding of the issues that can affect people as they age. Additionally, you have: A degree in Social Work, Psychology, Counselling, or Behavioural Science. Experience providing support, education, or group work to individuals and families. Strong understanding of issues affecting older Australians, including social isolation and elder abuse. Excellent interpersonal and conflict resolution skills.
