This role is central to supporting older people's recovery and independence following a hospital stay, while helping manage hospital demand and patient flow. As a TCP Care Coordinator, you will coordinate admissions to the Transition Care Program, develop person-centred care plans, monitor progress, and facilitate safe discharges in collaboration with General Practitioner's and the multidisciplinary team. You'll work across both hospital and community settings, supporting clients and carers to achieve optimal health restoration and independence. Key Responsibilities Coordinate TCP admissions and discharges to ensure optimal bed occupancy and patient flow. Develop and review individualised care plans and goals. Provide care coordination and support in both ward-based and home settings. Collaborate with the multidisciplinary team to promote recovery and successful transitions. Act as a central point of contact for clients, carers, and service providers. About you Experience in aged care, care coordination, or hospital discharge planning. Strong communication and interpersonal skills. Ability to work collaboratively across teams and services. Commitment to person-centred care and promoting independence.
