Locum Case Manager – South Morang Join a not-for-profit organisation making a real difference in the lives of young people. We are seeking an experienced Case Manager for a short-term locum role, supporting young people with complex needs who are at risk of homelessness. This is a full-time position for 1–2 months, based in South Morang, with the opportunity to transition into a permanent ongoing role . Key Responsibilities: Provide case management and support to young people experiencing or at risk of homelessness. Develop and implement individualized care plans. Coordinate with internal teams, external services, and community organisations. Maintain accurate client records and reports. Essential Requirements: Previous experience in case management or a similar role. Strong understanding of youth support services and housing pathways. Excellent communication, interpersonal, and organisational skills. Ability to work independently and as part of a multidisciplinary team.
