Your role at St John of God Social Outreach We are seeking applications for a Support Coordinator to join our Young Parents & Baby Program in Daniella, Perth, on a full-time permanent basis. As Support Coordinator, you will provide a high standard of care and quality service to the residents of the St John of God Social Outreach Horizon House, by empowering the young people to move to a position of autonomy and self-fulfilment by assisting them to achieve their goal and encouraging and facilitating involvement of the residents in education, training and personal development programs. The Position Provide safe and stable long-term accommodation to a young people who are at risk of being homelessness. Able to provide outreach, therapeutic and practical support to meet the continuum of care arrangements for young people with complex needs. Assist young people to realise their aspirations and potential integration into mainstream society. Provide support to young people in areas including independent living skills, involvement in education, training and employment services. Enhance a young person’s wellbeing and personal development through engagement with the broader resources and supports available in the community. You will have a well-developed understanding of trauma informed care including theoretical perspectives such as loss and grief, attachment, resilience and connectedness, child & adolescent development and the capacity to apply a range of practice responses. You must have a sound understanding of the challenges involved in service delivery to young people in the homelessness sector and experience in program coordination and leadership of a team to support program delivery. Successful applicants will hold a tertiary qualification in Youth Work, Social Work or Psychology and a minimum of 5 years’ experience working with young people in a residential care setting, current & valid first aid & CPR and full un-restricted Drivers Licence.
