The Aged Care Assessment Service is a Commonwealth funded program which has legislated delegation authority to approve clients for Residential Aged Care, Respite Care, Support at Home and Transition Care. The Aged Care Assessment Service Team Leader is responsible for compliance of the Australian Government Aged Care legislation and reforms, ensuring systems and resourcing to meet the KPIâs within the multidisciplinary team. Aged Care Assessment Service Team Leader also has a role as an Aged Care Assessment Service assessor, delegate, Lead Educator and the management of the triage process through the My Aged Care portal.
